Special note

	This form is only work for People's Significant Control declare by the 25% of shareholding, there is no any function for new shareholding registration
	The new shareholding also be the director, please use Form of Appointment of Director
	For new shareholding registration, please use Confirmation Statement to declare
	For the company needs to increase the share, please use Form of Share Allotment

Special note to customer

You need to disclose the following for 25% or more of shareholder and for public record:

	Name (same as Passport and foreign language name such as Chinese)
	Date of Birth (The day will not be disclosed)
	Nation (should for the definition of Companies House)
	Tax Resident Status
	Shareholder Service Address
	Tax Resident Registered Address and Actual Address
	Date of Registered as PSC
	Date of Record of the Date of Registered as PSC
	Other information should be submit

SERVICE NOTE AND REMARK

1.	The currency is in Sterling. Any one of director who are the residence in Northern Ireland, Great Britain, Isle of Man and Channel Island should pay the additional VAT 20%.
2.	The customer should provide the proof of identity and proof of address in Original or Certified True Copy for our due diligence purpose.
3.	Unless the application reject by us, the fee will not be refund when the Companies House reject because the problem status cause by the Director(s) or Shareholder(s).
4.	The due diligence should be done by us. If the customer cannot pass the sanctions list of the United Kingdom, the European Union and the United Nations, the individual or legal entity will not be able to apply for our company's services.
5.	The "Day" means "Working Day" and start from the next working day of Applicant Day, it is not including Saturday, Sunday, Great Britain Bank Holidays and Bad Weather Working Suspension Day issue by England Government. All working hour does not including the Transportation of Documents. All of the Documents should use Courier to delivery. For the Registered Mail will be delivery to outside of United Kingdom.
6.	Our Company only prepare, submit the documents that those information provided by applicant and collect the filed documents. If the application is rejected by government, we will no any responsibility to make refund or compensation.
7.	The fee schedule start from 22 April 2024, there will not be notified when any changes.
